- The Talking with Your Doctor Package was developed by the National Institute on Aging. This is a resource for patients who want to learn how to become better self-managers and how to become more active in their healthcare.

- Effective techniques for Patient Self-Management Video by California Healthcare Foundation
- To view the video, visit the California Healthcare Foundation Web site by clicking on the link above. This video provides three different demonstrations of patients learning to self-manage. This is a 30-minute presentation providing strategies and tools that have been proven effective in helping patients to self-manage and adopt healthy behaviors. This material is presented by William H. Polonsky, PhD, CDE., of the University of California, San Diego.

THREE COMMON MEDICATIONS TRIGGER EMERGENT CARE IN OLDER ADULTS: STUDY
December 4, 2007 -- Annals of Internal Medicine -- The Beers criteria is a consensus-based list of 41 potentially inappropriate medications for the elderly. However, it is three medications outside of this list – warfarin, insulin, and digoxin – that account for 33 percent of adverse events leading to emergency department visits by older Americans, according to researchers from the Centers for Disease Control and Prevention.
