Home Telehealth Disease Management Series

 

The Home Telehealth Disease Management Series provides clinicians with a package of tools to improve the management of heart failure, chronic obstructive pulmonary disease (COPD), cancer, and diabetes through the incorporation of telehealth technology. Each disease topic includes Patient Selection Criteria, Decision Support Tools, Patient Encounter Documentation Tools, Patient Self-Care Workbooks and Staff Education Guides. These tools may be easily adapted for the non-telehealth patient population as well. They are located on www.medqic.org.

Home Health Prospective Payment System Fact Sheet
Centers for Medicare & Medicaid Services (CMS) enterprise applications are those hosted and managed by CMS and do not include Fiscal Intermediary (FI)/carrier/Medicare Administrative Contractor (MAC) Internet applications. This one page guide, released December 2007, provides instructions for registering two staff members for the IACS-PC CMS online application.
