TIP OF THE WEEK

Communicate efficiently using the SBAR technique

 

SBAR is a structured communication technique designed to convey a great deal of information in a succinct and brief manner. This is important as we all have different styles of communicating, varying by profession, culture and gender.

 

SITUATION

A concise statement of the problem: what is going on now. For example, “Mr. Smith is experiencing a five on the ten-point pain scale focused on his right knee, which is an increase of three points since yesterday.”

 

BACKGROUND

Pertinent and brief information related to the situation: what has happened. For example, “Mr. Smith had a total knee replacement and was transferred to our facility two weeks ago for rehabilitation.”

 

ANALYSIS

Analysis and considerations of options: what you found/think is going on. For example, “I suspect that the increased activity influenced this change.”

 

RECOMMENDATION

Request/recommend action: what you want done. For example, ”Could you write an order for PRN pain meds to be given before therapy to head off any discomfort from the increased activity?”

Essential Systems for Quality Care for Pressure Ulcers

Key systems addressed in this summary table are the organization, screening, assessment, and plans of care for pressure ulcers. Key interventions include building interdisciplinary pressure ulcer teams or workgroups, standardizing pressure ulcer risk screening and monitoring, and designating accountability and oversight for all care plans, including those related to pressure ulcers.

MARCH IS NATIONAL NUTRITION MONTH

March 2007 -- CMS -- March is National Nutrition Month(r) - Please join with the Centers for Medicare & Medicaid Services (CMS) in promoting increased awareness of nutrition, healthful eating and the medical nutrition therapy (MNT) benefit covered by Medicare. Approximately 8.6 million Americans1 at least 60 years or older are diagnosed with diabetes or acute renal failure. MNT provided by a registered dietitian or nutrition professional may result in improved diabetes and renal disease management and other health outcomes and may help delay disease progression.

 

Medicare provides coverage of medical nutrition therapy (MNT) for beneficiaries diagnosed with diabetes or renal disease (except for those receiving dialysis) when provided by a registered dietitian or nutrition professional who meets the provider qualifications requirement, or a "grandfathered" dietitian or nutritionist who was licensed or certified as of December 21, 2000. A referral by the beneficiary's treating physician indicating a diagnosis of diabetes or renal disease is required. Medicare provides coverage for 3 hours of MNT in the first year and 2 hours in subsequent years. Read more >>
