Celebrating 10 Years of Quality Improvement
Medications are an important component of patient health. When taken correctly, they can greatly enhance patients’ overall health and well-being, especially for people suffering from multiple chronic conditions. Prescription, over the counter and herbal medications are all part of the complete medication profile.
Ohio KePRO, the Medicare Quality Improvement Organization (QIO) for Ohio, is working under contract with the Centers for Medicare & Medicaid Services (CMS), to identify community pharmacists and other healthcare professionals to reduce the incidence of drug-drug interactions (DDI) and potentially inappropriate medications (PIM) in people aged 65 and older through medication therapy management (MTMS) services. This project will run through December 31, 2010, and is part of the U.S. Department of Health and Human Services (HHS) Health Resources & Services Administration’s (HRSA) Patient Safety and Clinical Pharmacy Services Collaborative (PSPC).
Contact Dianne Miller at Ohio KePRO at 1.800.385.5080 or dmiller@ohqio.sdps.org for more information about participating in this quality improvement initiative.
