Medication events related to medication therapy occur all too frequently, and open the door for preventable harm due to medication errors. Adverse drug events disproportionately affect senior patients across the spectrum of care, including in hospital, ambulatory care and long term care settings. Patient safety and quality of care are negatively impacted by chronic disease conditions and a lack of coordinated care across care settings and among care providers negatively effects patient safety and the quality of care received.

The Adverse Drug Events project is a patient-focused, community-based effort aimed at improving medication safety to improve health outcomes in high-risk patients as a result of polypharmacy, and to improve health outcomes by reducing Emergency Room visits, hospitalizations and readmissions. The project involves collaboration among healthcare providers and facilities, community organizations, and patients and their caregivers to maximize use of clinical pharmacy services and to spread safe medication practices to promote improved patient outcomes.
