Ohio KePRO has partnered with primary care practices throughout the state to further
the goals of the Medicare-funded Prevention Electronic Health Record (EHR) Initiative.
From 2009-2011, our quality improvement specialists will work with targeted practices
to maximize the capabilities of their EHR systems in delivering patient care by
increasing efficiency, engaging patients in self-care, and proactively evaluating
practice performance in four preventive care measures: breast cancer screening,
colorectal cancer screening, and influenza and pneumococcal immunizations.
Of the nearly 100 practices involved in this project, 62 are active participants
and 35 are acting as a “reference group” to set the benchmarks for project performance.
As part of the Initiative, our quality improvement specialists will visit with participants
and provide:
-
A comprehensive assessment of office workflow and EHR reporting capabilities
-
Care management education and training
-
Review of quality data reports for accuracy
-
Assistance in identifying focus areas for improvement
-
Appropriate clinical topic interventions
-
Implementation of follow-up protocols, and
-
Identification of community-based services for patients to receive free or low-cost
screenings and/or immunizations.
