The goal of Improving Health for Populations and Communities is improve the overall health for populations and communities through primary care prevention and early diagnosis. Ohio KePRO is partnering with healthcare providers, patients, public agencies, and other groups and individuals to promote improvements in quality and produce care that is more integrated and more patient-centered, resulting in a healthier population. Our focus is on working with healthcare practitioners to:
- Create a healthier population by promoting and providing preventive services to more patients in key areas.
- Immunizations and cancer screenings
- Cardiovascular health
- Improve care by integrating health information technology to achieve meaningful use and improve care coordination and prevention.
- Increase of successful Physician Quality Reporting System (PQRS) reporting
- Collection, analysis and sharing of clinical data to reduce cost and improve care
