- Acute Myocardial Infarction (AMI)
AMI-1: Aspirin prescribed at arrival (no transfer exclusion)
AMI-2: Aspirin prescribed at discharge
AMI-3: ACEI or ARB for LVSD (no transfer exclusion)
AMI-5: Beta blocker prescribed at discharge
AMI-6: Beta blocker prescribed at arrival (no transfer exclusion)
AMI-7: Time to thrombolytics (no transfer exclusion) TEST MEASURE ONLY
AMI-New rural measure: Time to EKG (no transfer exclusion)
- Heart Failure (HF)
HF-2: LVF Assessment
HF-3: ACEI or ARB for LVSD
- Pneumonia (PN)
PN-1: Oxygenation assessment
PN-2: Pneumococcal vaccination
PN-5b: Initial antibiotic received within 4 hours of hospital arrival

Protocol for emergency department transfer communication. A composite of the following items:
- Nurse communication with receiving hospital
- Doctor communication with receiving hospital
- Patient Name
- Patient Address
- Patient Age
- Patient Gender
- Patient's Significant other's contact information
- Vital signs
- Physician history and physical: includes physical exam, history of current event, and chronic conditions.
- Physician orders and plan
- Nurse documentation: includes assessment/interventions/response
- Medication administration, allergies, adverse events
